Ocena funkcji biodra po leczeniu złamań przezkrętarzowych kości udowej z użyciem śrubopłytki DHS
Abstract
Between January 2000 and December 2003 54 cases of pertrochanteric femoral fracture were treated with DHS at the Department of Orthopedic Surgery in the Medical University of Gdańsk. An evaluation of results of treatment was made on the basis of 31 cases. We evaluated the patients age, mechanism of trauma and long term results of treatment. According to Boyd and Griffin classification 45% of type 2, 39% of type 1, 9% of type 4 and 7% of type 3 fractures occured. On the basis of D’ Aubigne – Postel Functional Value of Hip Scale we reached 90.1% of good and very good results of treatment; we think it is a gratifying result. A conception of intraoperative measurement of TAD (Tip Apex Distance) preventing screw cut – out was presented. Conclusions: treatment of pertrochanteric fractures of the femur with the application of DHS proves to be a stable and reliable fixation, that enables early mobilization of patients and weightbearing of the operated limb. A full range of DHS screws is necessary for a correct fixation. A short time of the operation and little intraoperative trauma in the patient allows the use of this method in patients with relatively high operative risk due to old age and systemic diseases. Therapeutic success is also conditioned by suitable rehabilitation.
