Wczesne pogorszenie retinopatii cukrzycowej w cukrzycy typu 1 po włączeniu intensywnej insulinoterapii

Abstract

The aim of his study was to describe “early worsening” of diabetic retinopathy in diabetes type 1 patients after very intensive insulin treatment. Material based on type 1 patients from Regional Diabetes Center with rapidly progressing retinopathy and rapid improvement of metabolic control. Fluorescein angiography, laser panretinal photocoagulation, pars plana vitrectomy and cryotherapy was performed. In one patient macular edema and proliferative retinopathy developed, in second patient developed ischemic maculopathy and proliferative retinopathy and was associated with serious visual loss.

Conclusion: According to DCCT it is observed an early worsening of retinpoathy in type 1 patients with rapid improvement of glycemic control. The monitoring before and during intensive treatment is important for visual acuity in log-term observations and for effects of treatment.
